There is a State of Illinois law that requires all students in kindergarten, second, and sixth grade to have a dental examination by May 15th of the school year. The dental exam may be dated up to 18 months prior to the May 15th deadline. Attached is a dental examination form for your convenience. Please return the completed form to the school nurse. If you feel that this requirement provides a financial burden to your family, please notify the school nurse for assistance with available resources. If you have already sent this information into school, kindly disregard this notice.
